This documents sets out the terms and conditions that apply to the participation in the monthly prize draw. The Midcounties Co-operative Limited reserves the right to change these terms at any time.

The prize draw promoter is The Midcounties Co-operative Limited, Warwick Technology Park, Gallows Hill, Warwick, CV34 6DA, UK (“Promoter”)

How to participate;

1. To take part, you must complete and submit the survey entry form.

2. This prize draw is free to enter.

3. There will be two winners per calendar month. The prize is a £100 Your Co-op gift card for each winner, which is redeemable in any Food store operated by The Midcounties Co-operative Limited. Gift card terms and conditions can be found on the back of the gift card packaging.

Restrictions;

4. The draw is open to UK residents aged 18 years or over, excluding employees of the Promoter or their immediate families or agents.

5. The Promoter accepts no responsibility for data corruption, incomplete or illegible entries, or entries which fail to be properly submitted.

6. Only 1 survey entry is allowed per person, per month. This means that no matter how many surveys are completed, only 1 of the responses will be put forward for the Prize Draw each month.

7. You will only be eligible to win one prize in any 12 week period.

The Prize;

8. There will be one prize draw per calendar month, and two winners per draw. The prize draw will take place within the first week of each calendar month. Entries received after the end of the month will be entered into the next calendar month’s draw.

9. The winner will be chosen at random by the Promoter from all completed and valid survey entries.

10. Prizes must be accepted as offered and are non-transferable.

11. No cash alternative is available.

Notifications;

12. Winners will be notified by phone or email on the day following the prize draw. The prize must be claimed within 5 working days of receiving the notification. If the prize is not claimed for whatever reason, a redraw will take place. The Promoter's decision is final and the Promoter shall have no liability to the original winner.

13. Winners will be sent their prize to their home address via the post. If the Promoter is unable to contact a winner or the prize is not collected, an alternative winner may be drawn at random and the Promoter shall have no liability to the original winner.

14. Each Prize Draw will be administered by The Midcounties Co-operative Limited and by completing a survey entry form you agree to The Midcounties Co-operative Limited using your details for the purposes of administering each Prize Draw you enter.

16. By completing a survey entry form, you agree to be bound by these terms and conditions. The Promoter reserves the right to refuse entry or refuse to award the prize to anyone in breach of these terms and conditions. The Promoter reserves the right to hold void, cancel, suspend or amend the promotion where it becomes necessary to do so.

Data Protection:

17. The Promoter will never release your personal details to any company outside of The Midcounties Co-operative for mailing or marketing purposes. The Midcounties Co-operative will use your details for the purpose of administering the Prize Draws only.

18. Winners may be asked to partake in publicity surrounding their win although this is not compulsory.

19. The Prize Draws will be governed by English Law and entrants to the Prize Draws submit to the jurisdictions of the English Courts.
